Diabetes Caregiver Organizer (Digital Download)

One spreadsheet for everyone who helps care for someone with diabetes. A parent, a spouse or partner, an adult child caring for an aging parent, a grandparent, a sitter or in-home aide, school or day program staff: everyone logs in the same place, and you print a handoff sheet whenever someone new steps in.

Fill it in once. Print a fresh Sitter, School / Day Program, Travel, Respite or Emergency sheet any time.

Light or dark, your choice

You get two versions of the same organizer: bright Daylight and a free Midnight dark mode that's easier on the eyes for late-night readings. Use whichever you like. Print from the Daylight file.

What's inside

  • Diabetes Caregiver Organizer spreadsheet (.xlsx) for Excel and Google Sheets
  • FREE: Midnight dark-mode version of the spreadsheet (same features)
  • Printables (one .zip file): 11 pages in US Letter, A4 and Low-Ink black and white
  • Quick-Start Guide (PDF)
  • Blank copies (Daylight and Midnight) for a one-step fresh start

Spreadsheet tabs (17)

  • START HERE: 3-step setup, privacy tips and the notice
  • Dashboard: 6 live tiles and 4 charts with a 7, 14, 30 or 90 day picker: readings over time, by caregiver, by location, and within or outside the range you entered from your care team (blank until you enter it), plus a "coming up" list of renewals, expirations, device changes, visits and follow-ups
  • Settings: first name only, mg/dL or mmol/L, the range your care team gave you (blank by default), caregivers, locations and kit locations
  • Quick Log: phone-first, 6 columns: date, time, reading, logged by, where, note
  • Shared Care Log: reading, food, carbs as counted, insulin or medicine exactly as given, who gave it, logged by, where. No dose, ratio or correction math and no totals
  • Handoff Builder: about the person, contacts, daily routine, where things are kept, blank boxes for your care team's steps, and a "when to text or call me" agreement
  • Sitter, School / Day Program, Travel and Respite Sheets: one page each, filled in for you
  • Emergency Info: a fridge and go-bag page plus a fold-in-half wallet card
  • Care Plan Papers: DMMP, 504, IHP, emergency, facility and day program plans, with version date, renewal date, status and who has a copy (not legal advice)
  • Supply Kits: home, nurse / office, classroom / day program, bag, car: quantity, expiry date, use-by after opening, reorder date
  • Device Changes: sensor, pump site or patch changes, next change from the wear days you type in
  • Sick-Day Log, Meetings & Messages, and Appointments

Enter once, print many

Type contacts, the routine and where supplies are kept one time in the Handoff Builder. The Sitter, School / Day Program, Travel, Respite and Emergency sheets fill themselves. Change a phone number once and every sheet is up to date. Each sheet prints on one page, Letter or A4.

Your care team's steps, never ours

Every sheet has clearly labeled blank boxes for lows, highs, overnight, before activity, sick days and when to call 911. You copy them word for word from the written care plan. We don't write care steps, suggest targets or fill in ranges.

Printables (11 pages)

Sitter Handoff Sheet, School / Day Program Sheet, Travel Checklist, Supply Kit Checklist, Sick-Day Log, Shared Weekly Log with "logged by", Emergency Info, Wallet Cards, Meeting & Message Notes, When to Text or Call Me, and First Week Setup Checklist. Each in US Letter, A4 and Low-Ink.

How it works

  1. Check out. Your download link appears right after checkout and in your order confirmation email, so you can download instantly.
  2. Open the .xlsx in Excel, or upload it to Google Drive and choose File > Save as Google Sheets.
  3. Fill in Settings and the Handoff Builder once, then log and print.

Privacy

Your file, your Drive, no account. The organizer lives on your computer or in your own Google Drive; we never see it. There are no fields for SSN, insurance ID or date of birth. Share a printed page or a PDF of one sheet, never the whole file.

Compatibility

  • Google Sheets (free) on a computer, phone or tablet
  • Microsoft Excel 2016 or newer, Windows or Mac, and Excel for the web
  • No macros, no scripts, no add-ons
  • Not built for Apple Numbers

FAQ

Who is it for?
Any adult caring for someone with type 1 or type 2 diabetes: a partner, a parent, an adult child, a grandparent, a sitter or aide. School and day program pages are one section alongside home, sitter, travel, respite and sick days.

Does it tell me what to do about a reading?
No. It records and organizes. There are no alerts, no dose math and no targets. Counts use only the range you type in from your care team.

Can several people log at once?
Yes. Share your Google Sheet by email with the people who help. Everyone adds rows to the Quick Log from their own phone.

mg/dL or mmol/L?
Either. Pick it in Settings. Readings accept decimals. Nothing is converted.

Log and organize only. Not medical advice. This organizer doesn't replace your care team's written plan.
